To add or edit questions on a form, open the form in the form editor, use the plus button in the questions card to add new questions, or click an existing question to edit it. Reorder with the drag handle and click Save form in the top-right when you're done.
Steps to add a question
- Open the Forms page from the sidebar.
- Click a form to open the form editor (or create a new one).
- In the questions card, click the plus button (its tooltip is Add question) in the top-right.
- In the Add question modal, select a question type from the left panel.
- Fill in the question text, toggle Is this a required field? if needed, and configure any type-specific settings (options, body metric, or required poses).
- Click Add question in the modal footer.
- Click Save form in the top-right of the form editor to save your changes.
Question types
These question types are available in the left panel of the Add question modal:
- Text — a written response.
- Number — a numeric value.
- Body Metric — syncs the client's answer to their body metrics. Not available on public forms.
- Drop Down — client picks one option from a dropdown.
- Multiple Choice — client picks one option from a list.
- Yes/No — a yes or no choice.
- Scale — a scale from 1 to 10.
- Date — a selected date.
- Star Rating — a star rating from 1 to 5.
- Progress Photos — client uploads progress photos, optionally tagged Front, Side, or Back. Not available on public forms.
- Media — client attaches a media file.
- Signature — captures a signature, useful for waivers and agreements.
Editing and reordering questions
- To edit a question, click its row in the questions list. The Add question modal reopens with its details, and the footer button changes to Update.
- To reorder questions, drag the 6-dot handle in the Order column up or down.
- To remove a question, click the trash icon on its row.
- All changes are buffered until you click Save form, so nothing is saved until you do.
Configuring options for Drop Down and Multiple Choice
When you pick Drop Down or Multiple Choice, the right panel shows an options list:
- Type each option into a row (placeholder shows Option 1, Option 2, etc.).
- Click Add new to add another option.
- Click the trash icon on a row to remove that option.
- Empty option rows are filtered out automatically when you save the question.
Configuring a Body Metric question
When you pick Body Metric, the right panel shows a metric selector. Pick which body metric the question collects (weight, body fat, waist, sleep, water intake, custom metrics, and more). When a client answers this question, their Body Metrics entry updates automatically.
Configuring a Progress Photos question
When you pick Progress Photos, the right panel shows three pose toggles: Front, Side, Back. Selected poses are required and get tagged in Progress Photos for easy comparison. If none are selected, clients can upload any photos.
Things to watch out for
- Body Metric and Progress Photos are hidden from the question type list when you're editing a public form — they only appear on check-in and one-time forms.
- Question changes (adding, editing, reordering, deleting) aren't saved until you click Save form in the top-right of the form editor.
- When editing an existing question, the modal footer button reads Update, not Add question.
- For a Body Metric question, you must select a metric before you can save the question — the save button stays disabled otherwise.
