Your Forms page holds every form you've built. A form is invisible to everyone until you assign it or share its link, so you can build it in peace.
Forms are created on the web dashboard. The coach mobile app opens the same dashboard, so everything here works there too.
Create a form
- Go to Forms and click New form.
- Enter a form name — up to 100 characters, for example "Weekly Progress Check-In".
- Pick the type: Check-In, Public, or One-Time.
- Click Create form. The form editor opens so you can add questions and agreements.
The type can't be changed after creation, so if you pick wrong, create a new form.
Add a description and cover image
Click the pencil on the form's hero to set a description (up to 500 characters) and a cover image. Both appear at the top of the form when it's opened.
Clients linked to a check-in template see name, description, and cover changes as soon as you save. Per-client copies are unaffected.
Preview the form before you save
The editor shows a live mobile preview beside it, stepping through the form exactly as your client or lead will see it. Nothing saves until you click Save, and you're warned if you navigate away with unsaved changes.

Find a form in your library
The Forms page searches by form name and lists each form's type, question count, assigned-client count, schedule, and reminder time, ten per page. Watch tutorials on the Forms and Form Submissions pages opens video walkthroughs.
Duplicate a form
Open the form's row menu and choose Duplicate. You get "{name} (Copy)" with the same questions, schedule, and agreements. Assigned clients and the cover image are not copied.
Delete a form
Open the form's row menu and choose Delete, then confirm. Forms are deleted one at a time.
The form leaves your library and its check-in reminders stop. Clients' past submissions are kept and stay visible, and per-client custom copies keep running until you delete them from each client's profile.
Things to watch out for
Creating, editing, and deleting forms require an active subscription — trial or paid. If yours lapses you can still view your forms and submissions.
A form with no questions can't be selected when you add a form from a client's profile. Add at least one question first.
