You can assign a check-in form to a client in two places: from the Forms page (fastest for adding the same form to many clients), or from the client's profile under the Forms tab (lets you choose whether the client gets a synced copy or an independent one).
Assign a check-in form from the Forms page
Use this when you want to give the same master form to one or more clients. Clients added this way are linked to the master template, so any edits to the master will sync to them.
- Go to the Forms page.
- Find the check-in form you want to assign and click the three-dot menu on its row.
- Click Assign form.
- In the Add clients to form modal, select one or more clients.
- Click Add clients.
Assign a check-in form from the client's profile
Use this when you want to choose between linking the client to the master form or giving them their own independent copy that you can edit just for them.
- Go to the Clients page and open the client.
- Open the Forms tab.
- Click Add Form.
- Under Form Type, select Check-in form.
- Under Assignment Type, choose one:
- Link to master — links the form to the master template. Any changes to the master automatically sync to this client.
- Create a copy — creates an independent copy of the form for this client that you can customize without affecting the master.
- Under Select Form, click the form you want to assign and review the questions on the right.
- Click Add Form.
The form will appear in the client's Assigned Forms list with a Linked or Custom badge depending on the assignment type you chose.
How to unassign a form from a client
Open the client's profile, go to the Forms tab, and use the action button on the form row:
- If the form is Linked, click the unlink icon and confirm Unlink client. This removes the form for this client only and does not delete the master form.
- If the form is Custom (an independent copy), click the trash icon to delete it for that client.
Things to watch out for
- The Forms page three-dot menu only links clients to the master form — it does not offer a copy option. To give a client their own independent copy of a check-in form, use the Add Form flow from the client's Forms tab and choose Create a copy.
- A form must have at least one question before you can assign it. If a form has no questions, you'll be prompted to edit the form and add a question first.
- For check-in forms, the recurring schedule is set on the master form itself — assigning the form does not change its schedule.
- One-time forms work differently: from the Forms page, the row menu shows Copy to clients instead of Assign form, because one-time forms always create an independent copy per client (there is no link-to-master option for one-time forms).
