To create an onboarding form, build a One-Time Form on the Forms page. One-Time Forms are designed for onboarding — they aren't on a recurring schedule, and they can be attached to an onboarding flow or automation so new clients receive them automatically.
Steps to create the onboarding form
- Open Forms in the sidebar.
- On the Form Templates tab, click New form.
- In the Create form modal, enter a form name (up to 100 characters).
- Under Select form type, choose One-Time Form.
- Click Create Form.
- In the form editor, add the questions you want clients to answer.
- Click Save form.
To edit the form later, open the Forms page and click the form's row in the Form Templates table.
Send the form to new clients automatically
Once the One-Time Form is saved, you can have it sent to clients without manually assigning it each time:
- Onboarding flows: Add the form to an onboarding flow so it's delivered when a new client account is created.
- Automations: Add the form to an automation to trigger it based on events you define.
You can also assign a One-Time Form to a client manually from their client profile after their account exists.
Choosing the right form type
- One-Time Form — submitted once. Use this for onboarding intake.
- Check-In Form — submitted on a recurring schedule (e.g., weekly check-ins).
- Public Form — shareable via public link for leads or surveys.
Things to watch out for
- A form's type can't be changed after it's created. If you pick the wrong type, create a new form with the correct type.
- Form names are limited to 100 characters.
- For onboarding, always pick One-Time Form — Check-In Forms will keep prompting clients on a schedule, which isn't what you want for an intake form.
