A public form is filled from a shareable link — no account needed. It's the form type for lead capture and intake, where the person answering isn't your client yet.
Public forms are created and shared from the web dashboard.
Share a public form's link
- Create a form with the Public type, add your questions, and save.
- Open the form and use the link card to copy the shareable link — or use Copy link from the form's row menu on the Forms page.
- Send the link anywhere: your website, socials, or email.
Use Preview form from the row menu to see the live page yourself before you share it.
What the public page looks like
The page carries your branding — your logo, business name, and theme color. Submitters must pass a human-verification check before their answers go through.
Choose what happens after a submission
In the public form editor's post-submission card, pick one:
- Custom message — a thank-you message shown on the success screen, up to 500 characters.
- Redirection — send the submitter to a URL of yours, like a booking page. Up to 2,000 characters, http:// or https:// only.
The redirect wins when a URL is set.

Review public submissions
Public submissions arrive in the Public tab at Forms → Form Submissions, alongside your check-in and one-time submissions, with its own pending count.
When someone submits, a "Public Form Completed" notification and an email go to whoever created the form.
Public submissions are reviewed or flipped back to pending, but there's no comment step — the submitter has no account to notify.
Things to watch out for
Body Metric and Progress Photos questions aren't available on public forms. Those answers write into a client's body metrics and progress photos, which need a client account.
If your subscription lapses, the public page stops accepting submissions and visitors see "This coach isn't accepting new form submissions right now." Existing submissions stay visible on your dashboard.
The form type can't be changed after creation, so a check-in or one-time form can't be turned into a public one. Create a new public form instead.
